Below are the forms that committees must use to register and file with the City Clerk's Department. Per A.R.S. § 16-926(A), campaign finance forms are prescribed by the Arizona Secretary of State's office. Completed forms may be submitted electronically to the City Clerk's Office by emailing them to Clerk@YumaAZ.gov.

• Campaign Finance Political Action Committee Guide 
• Termination Statement - To be used to terminate a committee
• Financial Disclosure - To be used by elected officials and candidates
• A Guide for Completing Financial Disclosure Statements
• Candidate Statement of Interest - a statement that is required to be filed by a person prior to obtaining a signature on a candidate nomination petition.
